Position Description
NURSE PRACTITIONER
JOB DESCRIPTION:
The Nurse Practitioner, in collaboration and consultation with physicians, staff RN's and other health care professionals provide holistic, compassionate care to patients and families. The Nurse Practitioner provides leadership by serving as clinical expert, educator, and liaison between the patients and the medical, nursing and interdisciplinary team with the goal of providing state of the art Palliative Care.
JOB QUALIFICATIONS:
- Possession of current license to practice as a nurse practitioner in the state of Oklahoma
- Minimum two years experience of varied work experience as a nurse practitioner and preferably one year in hospice setting
- Demonstrate ability to assess and respond to the needs of patients and families in varied settings
- Demonstrate skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles
- Demonstrate understanding and acceptance of hospice principles of care
CRITICAL TASKS:
- Provide direct oversight of care to home-based patients and residents of long term care facilities with chronic and terminal illnesses
- Perform comprehensive health assessments to promote quality of life and a symptom free experience with patients and their families
- Obtain histories, perform medical examination, diagnostic evaluations, pharmacological interventions and collaboration related to symptom management recommendations
- Obtain data on physical, psychological, social and spiritual factors that may influence patient/family health status and incorporate that data into the plan of care
- Participate in interdisciplinary meetings with emphasis on the medical management of patients' plans of care
- Inform the interdisciplinary team of unusual or potentially problematic patient/family issues
- Offer advice and information to staff and referring physicians on medical interventions consistent with hospice philosophy and the plan of care
- Assist the medical director and patient care coordinator with the establishment of appropriate health policies and requirements for the hospice staff
- Consult with attending physicians and hospice staff as requested so that pain and symptom management for patients is effectively and expeditiously accomplished
- Serve as medical liaison with the medical community and promote referrals
- Participate in community programs for the purpose of providing education and information to members of the medical community
- Demonstrate familiarity with policies of the agency and rules and regulations of State and Federal bodies which aid in determining policies
- Maintain strict confidentiality regarding all organization matters, patient information, and contents of hospice medical records
- Other assignments as may be appropriate
